Rules That Apply
The organizer of the “Zdravlje Plus – Together for Health” Loyalty Program (hereinafter: Loyalty Program) is Oktal Pharma d.o.o., Utinjska 40, Zagreb (hereinafter: Organizer). Membership in the Loyalty Program is free of charge.
A person becomes a member of the Loyalty Program by completing and submitting a printed application form at any Zdravlje Plus pharmacy sales point or by filling out an online application form at selected web locations. The content of the application form is determined by the Organizer, in accordance with the law and the needs of the Loyalty Program. The application form must include the member’s confirmation of acceptance of these Rules in their current version. By accepting the Rules, the member also consents to the collection and use of their personal data, submitted upon enrollment, for the purposes and in the manner defined by these Rules. Upon enrollment, the member receives a Loyalty Program card with a membership number, which entitles them to membership rights as defined by these Rules. Only individuals over the age of 18 can become members of the Loyalty Program.
Membership in the Loyalty Program does not impose any obligations on the member. By enrolling, the member gains the right to participate in the Loyalty Program, which includes:
By signing and checking the box “I agree with the proposed processing of personal data” on the application form, members of the Zdravlje Plus Loyalty Program entrust their personal data and consent to its collection and processing for the purpose of implementing the Loyalty Program. Oktal Pharma d.o.o., Utinjska 40, Zagreb, is responsible for data processing.
The purpose of data processing is to enable the use of Loyalty Program benefits and related marketing activities (e.g., notifications about promotions and new products), as well as maintaining records of medicines and other products used by the member to ensure health and prevent interactions between medicines and other products.
The Organizer will link the member’s profile with purchase data made using the membership card, in order to better understand member needs and preferences, prepare offers, and provide information about products and services that may be of interest. The following personal data is used: name and surname, home address, gender, date of birth, OIB and/or MBO, phone/mobile number, and email (hereinafter: Personal Data).
Data is not sold to third parties or commercialized in any other way. To protect Personal Data from accidental or unlawful destruction, loss, alteration, unauthorized disclosure, or access, the Organizer uses technical and organizational security measures. The Organizer shares Personal Data with pharmacies participating in the Zdravlje Plus – Together for Health program (hereinafter: Zdravlje Plus pharmacies) for the purpose of providing services and benefits to Loyalty Program members (a list of all Zdravlje Plus pharmacies is available at www.oktal-pharma.hr).
Additionally, the Organizer may share Personal Data with trusted partners who maintain IT systems or provide services on behalf of the Organizer (e.g., for marketing, financial, or advertising purposes). These service providers are contractually obligated to use the entrusted data exclusively in accordance with the Organizer’s guidelines and strictly for the stated purpose. They are also required to properly protect all Personal Data and keep it confidential. Any changes to Personal Data, as well as requests for access, correction, modification, or deletion, or exercising rights to restrict processing, data portability, or withdrawal of consent, must be submitted by the member to the Organizer via email to the Data Protection Officer at szop@oktal-pharma.hr or in writing to the Organizer’s address.
A member may cancel their membership by sending a written notice, without explanation, to the Organizer’s address or by deregistering at any Zdravlje Plus pharmacy sales point. The Organizer may also cancel a member’s membership by written notice, without explanation. Membership will in any case terminate upon discontinuation of the Loyalty Program in accordance with the Organizer’s decision.
Personal data of members will be deleted upon termination of membership in the Zdravlje Plus Loyalty Program for any reason. Remaining points on the card will be void and cannot be used or transferred to another card.
In case of loss or theft of the card, the member must report the loss at the nearest Zdravlje Plus pharmacy, via email at zdravljeplus@oktal-pharma.hr, or by phone at 01/6596 869. The Organizer will send a new card to the member’s address, transferring any previously earned and unused points.
These Rules take effect on the date of publication on the website www.oktal-pharma.hr. The Organizer is authorized to make amendments and supplements to these Rules or adopt new Rules at any time, without the need for member consent, which will apply from the date of their publication on the website www.oktal-pharma.hr.
